I, the undersigned, hereby acknowledge that I am voluntarily participating in adult men’s lacrosse games, practices, and related activities organized by 727 Lax LLC ("727 Lax"). I understand and agree that lacrosse is a physical, contact sport that carries with it inherent risks, including but not limited to serious injury, concussions, permanent disability, or death, as well as property damage. These risks may arise from my own actions or inactions, the actions or inactions of others, field conditions, equipment, weather, travel, or the nature of the sport itself.Concussion Awareness & Acknowledgment
I understand that participating in this sport involves a risk of concussion or other head injury. A concussion is a serious brain injury that may be caused by a bump, blow, or jolt to the head or body. Symptoms may not be immediately apparent and can lead to long-term brain damage if improperly managed. By checking the box below, I acknowledge that I have reviewed and understand the concussion safety information provided by the CDC at https://www.cdc.gov/heads-up/ or I have completed a similar concussion protocol training or awareness program. By playing in 727 Lax activities, I hereby accept the risk of concussions and I consent not to return to activity until cleared in writing by a licensed health care provider if I am diagnosed with a concussion. Furthermore, I agree to indemnify and hold harmless 727 Lax and its affiliates from any and all liability, costs, or legal actions (including attorney’s fees) resulting from claims related to head injuries, unless caused by gross negligence or intentional misconduct.Release and Indemnification
In consideration of being allowed to participate in 727 Lax activities, I, for myself, my heirs, assigns, and personal representatives, do hereby fully and forever release, waive, discharge, and covenant not to sue 727 Lax LLC, its officers, directors, managers, coaches, volunteers, affiliates, sponsors, referees, or any associated personnel or entities (collectively, “Released Parties”) from any and all claims, liabilities, demands, causes of action, or expenses (including attorney’s fees) arising out of or related to any injury, accident, illness, death, or property damage sustained during, or as a result of, participation in 727 Lax activities, regardless of whether caused by negligence or otherwise.
I further agree to indemnify, defend, and hold harmless the Released Parties from any and all claims brought against them by third parties that may arise from my conduct or participation in 727 Lax activities.Medical Treatment Authorization
I certify that I either have my own insurance coverage, have a USA Lacrosse Membership, or knowingly waive those rights, and accept full financial responsibility should anything happen to me (or my child if under 18). I hereby authorize 727 Lax, its coaches, officials, volunteers, or representatives to secure emergency medical treatment, including transportation by ambulance and evaluation by on-site medical staff if necessary. I acknowledge that I am solely responsible for any and all medical expenses incurred, regardless of insurance coverage, and further acknowledge that 727 Lax is not responsible for any costs associated with such medical care.Code of Conduct & Rules Compliance
I agree to abide by all rules, policies, and codes of conduct established by 727 Lax and its officials, including rules of play, sportsmanship, and respectful conduct toward teammates, opponents, referees, and spectators. I understand that failure to comply may result in disciplinary action, including but not limited to suspension or permanent removal from 727 Lax participation, without refund of fees.
Media Release
I grant permission to 727 Lax to use my (or my child’s) image, likeness, name, voice, and/or appearance in photographs, video recordings, digital images, or any other media taken during 727 Lax activities for promotional, educational, or informational purposes without compensation. I understand these materials may be used in print, online, or social media and will remain the property of 727 Lax.Infectious Disease / COVID-19 Waiver
I acknowledge that participation in 727 Lax activities may expose me (or my child) to communicable diseases, including but not limited to COVID-19. I voluntarily assume all risks related to such exposure and agree that 727 Lax, its affiliates, officers, volunteers, and the facility providers shall not be held liable for any illness, injury, or damages resulting from such exposure. I further agree to comply with any health and safety protocols implemented by 727 Lax.
Severability & Governing Law
If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain valid and enforceable to the fullest extent permitted by law.
This waiver shall be governed by and construed in accordance with the laws of the State of Florida, and any disputes shall be resolved in the courts of Pinellas County, FloridaCertification
If registering for the Men’s Open league, I certify that I am at least 18 years of age, physically fit, and sufficiently trained to participate in lacrosse activities.
If registering for the High School league, I certify that the player I am registering will be enrolled in high school during the 2025 school year, is physically fit, and sufficiently trained to participate in lacrosse activities.
I have read this waiver and fully understand its terms, including the risks of concussions. I understand that I am giving up substantial rights, including the right to sue, and I sign it freely and voluntarily.
